When you start comparing Medicare Advantage plans, you'll notice many of them carry three letters after their name: HMO or PPO. Those letters aren't just labels. They tell you how the plan's network works, whether you'll need referrals, and what happens when you see a doctor outside the plan.

Medicare's Annual Enrollment Period runs through December 7, and changes generally take effect January 1. If you live in Mooresville, elsewhere in Iredell County or around Lake Norman, understanding the plan type is one of the simplest ways to narrow your choices before you get into the finer details.

What an HMO plan generally means

HMO stands for Health Maintenance Organization. In a Medicare Advantage HMO, you generally get your care from doctors, hospitals and other providers in the plan's network. Some common features:

  • You usually choose a primary care doctor who coordinates your care.
  • Many HMO plans require a referral from that doctor before you see a specialist, though rules vary by plan.
  • Care from out-of-network providers usually isn't covered, except for emergencies, urgent care when you're away from home, and out-of-area dialysis.

Some HMO plans include a point-of-service (POS) option, which may let you get certain services out of network, often at a higher cost. If you see "HMO-POS" on a plan, ask exactly what that option covers.

An HMO can be a good fit for people who already use doctors in the plan's network and like having one doctor at the center of their care.

What a PPO plan generally means

PPO stands for Preferred Provider Organization. A Medicare Advantage PPO also has a network, but it gives you more room to go outside it:

  • You can see in-network providers at the plan's regular cost sharing.
  • You can usually see out-of-network providers too, typically at a higher cost.
  • You generally don't need a referral to see a specialist.

One detail people miss: an out-of-network provider doesn't have to agree to treat you, except in an emergency. Before you book with a doctor outside the network, it's wise to call and confirm they'll see you under your plan.

A PPO may appeal to people who split care between several towns, see specialists often, or want the flexibility to keep a doctor who isn't in a particular network.

What both plan types have in common

Whichever type you choose, a Medicare Advantage plan covers everything Original Medicare Part A and Part B cover. A few other points apply to both:

  • Emergency and urgent care are covered anywhere in the United States.
  • Every Medicare Advantage plan has a yearly out-of-pocket maximum for covered Part A and Part B services. PPO plans often show two figures: one for in-network care and a combined one for in-network and out-of-network care.
  • Many plans include Part D prescription drug coverage, but not all do, so check.
  • Extras like dental, vision or hearing may be included, but benefits vary by plan.

How the Lake Norman area factors in

Around Lake Norman, care often crosses county lines. You might see a family doctor in Mooresville, a cardiologist in Huntersville and a specialist in Charlotte, with a hospital stay possible in Statesville. That's why the plan type matters here. Before you choose, list every doctor, specialist and hospital you use and check each one against the plan's network for next year.

If you spend part of the year somewhere else, such as visiting family out of state or staying at a second home, think about how much routine care you'd need while you're away. An HMO generally covers only emergency and urgent care outside its service area. A PPO may cover out-of-network care elsewhere, but the details depend on the plan.

For a step-by-step way to check providers and prescriptions, see checking your doctors and pharmacies during Medicare enrollment. If you're still deciding between the two main paths, Original Medicare vs. Medicare Advantage explains how they differ.

Questions to ask about any plan

  1. Is this an HMO, HMO-POS or PPO, and what does that mean for the doctors I use?
  2. Do I need referrals for specialists?
  3. What does out-of-network care cost, if it's covered at all?
  4. What is the out-of-pocket maximum, in network and combined?
  5. Are my prescriptions and pharmacy covered?
